Health Tracks
Overview
Health Tracks is a medically
supervised program to help people improve their overall physical, mental
and social functioning. The goal is to stabilize, slow or even reverse
the progression of diseases, especially those with a lifestyle
component.
Program
components include:

Supervised exercise
Y Counseling on modifiable risk factors like nutrition, stress, or exercise, and chronic disease management
Y
Social support
The Health
Tracks program is offered Monday, Wednesday, and Friday in the morning.
Participants come for about an hour to exercise and participate
in one-on-one health education.
After consultation with their healthcare provider and the Health
Tracks nurses, people can opt to attend one, two or three times a week.
The Health
Tracks program is administered by nurses from the Cardiac Rehabilitation
team.
Health
Tracks is designed to be an affordable option for health maintenance and
improvement. Participants
must pay privately for 10 sessions at a time.
