Oakview Terrace

 

Oakview Terrace is a Medicare Skilled Long Term Care facility offering complete, round-the-clock nursing care.  It is staffed with Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants.  Oakview Terrace is physically attached to the hospital, with easy accessibility to 24-hour emergency facilities and medical staff. Physical, occupational and speech therapies, restorative care, acvtivities, dietary consultant, program, social services, and adult day services are offered as well.

In addition, care conferences are held to discuss each Oakview Terrace resident's plan of care.  A thorough assessment is  reviewed every three months for each resident; the care team meets weekly.  Staff involved in the care conferences include a registered nurse, restorative care aide, activities coordinator, social worker and dietary manager.  Family members as well as the resident are invited to participate in the meeting giving them an opportunity to express ideas and share concerns with staff.  Family members are encouraged to contact any member of the care team at any time with questions or concerns.

 

In 2005, a Dementia Care Unit opened at Oakview Terrace.  The 10-bed unit is a secure, warm and safe environment that revolves around the special needs of  people with Alzheimer's Disease.  The specially trained staff provides care to attain or maintain the highest physical, mental and psycho-social well-being of each resident.

 

The staff in the unit consists of Certified Nursing Assistants and Licensed staff.  The nursing staff emphasizes the "wholistic" approach, which includes caring for the physical, emotional, spiritual and psycho-social needs of each resident.  The care these residents receive is "person centered care," with an emphasis on activities.

 

The concept of  "person-centered care" allows staff to focus on each individual's unique set of life circumstances.  The resident's individuality and interests are taken into consideration when planning activities.  Staff allows each resident to set his or her own schedule in regards to eating, sleeping, or rising in the morning.  Staff is sensitive to the needs of each resident.
